Anyone have bcbs empire? Did they require a certain amount of iuis before moving on to ivf?
*please don't come at me. I know it "based off your plan" and "your diagnosis" and "call and ask". I'm asking for your experiences).

I had BCBS 2 years ago through my employer when I started fertility treatments, not empire though, but my specific plan was a fertility treatment lifetime $ cap (they would cover 10k towards fertility treatments 10k towards fertility meds) so with our diagnosis we decided to do 6 iuis (which failed for us and we needed to move to IVF the following year), but we could have gone straight to IVF if we wanted to. After that I switched to a different plan. It will for sure depend on your plan though because my friend had BCBS from a different employer who had 0 fertility coverage.

I have BCBS of MA (not empire though). They required 6 failed IUI’s before they would approve IVF. However...there’s ways around that. My RE accidentally 😉 had too many follicles grow and insurance didn’t consider it safe to do IUI with that many follicles in case they all released (10+)so they approved a switch from IUI to IVF on my second IUI cycle. I have PCOS for reference!
