I'm confused. If the same 12 clients see you every 3 months the total number of clients is 12. If the same 12 clients see you every 3 months, you see clients a total of 48 times a year. If you see 12 new clients every 3 months, you'd have 48 new clients in a year. I'm not sure what exactly you are asking. 
It would be 12x4 because 3 (months) goes into 12 (months/12mo in a year) 4 times.

You have 12 clients regardless of how often they see you. If you’re trying to count how many appointments you’ve seen, it’s number of clients x number of visits. So 12 x 3.
