Boyfriend has extreme orgasms

My boyfriend told me he has very intense orgasms, and I didn’t believe it was this bad. I didn’t believe him until he had one. He pulls out, like gasps for air, and like screams under his breath. Sometimes he beats his head into the pillow, or even swings. I get it, but I’m also thinking about the other side of things… could something medically be wrong?? He said it’s just sooo intense, but idk. I’ve never seen it this bad
