Sac on ultrasound

When did you guys start seeing a sac on your ultrasounds? I went to the ER for cramping I’m almost 4 weeks and no sac was seen so er told me I could be having a ectopic pregnancy or never was pregnant and all 5 of my tests gave me false positives
