Please help. I'm concerned

I've been on the combo pill (ocella) for 2 years (for birth control and to control my heavy periods). This past couple of months I've been having breakthrough bleeding and it's concerning. I bleeding after sex (bf uses condoms always) and now I bleeding after working out. I went to planned parenthood and they said it's normal and not pregnant. Went to my general doctor and said it was not urgent just annoying.

Can someone share stories, tips, suggestions, anything to help me understand what's going on.
