An EXTREMLY Heavy period… what does this mean?

So I would say for the past year I have started to have really heavy periods. Most of my life I was able to use a regular tampon but over the past year I’ve started having to use Ultra. For a while those were able to last me several hours , but now I bleed through those pretty quick and there is not a bigger size than ultra. Today, I woke up to the feeling of blood actually gushing out of me. Is it normal to get increasingly heavier periods after time (I’m only 21) or should I go see a doctor ? This is the picture I took:
