Really heavy periods

Hey I been having really abnormal periods like since the birth of my daughter my period has been really bad sometimes . Like one month I’ll bleed a little bit for a few days and the next I month I will bleed like a dam so bad I have to change my tampons every two hours or ill bleed thru and I’ll have to wear a pad also so it doesn’t and these tampons are the ultimate size. I been reading that it could be signs of pre-menopause but im only 39 ? what your guys take on it ?
