Ivf advice please! Stage 4 Endo

Really keen to get some opinions.

Background is we have been trying for our first baby for 1.5yrs, natural pregnancy on 7th cycle ended in chemical pregnancy, natural pregnancy on 9th cycle ended in missed miscarriage at 10 weeks. Scans found endometriomas on my ovary. Had surgery in March to remove the endometriomas and they found severe stage 4 endo and adenomyosis also.

Iv now had 2x cycles with letrozole and a scan found that 2x endometriomas have reformed on my right ovary. My fertility specialist now has us doing our first cycle of <a href="https://glowing.com/glow-fertility-program">ivf</a> in August to hopefully get some Embryos to freeze to preserve my fertility (the goal is 5 embryos), before doing an extensive surgery 2-3mths later to remove all the stage 4 endo and the endometriomas. Then once iv recovered do down regulation and then can do frozen embryo transfer (likely early next year by the time I have surgery and down regulation).

I totally understand that the amount of embryos you get from first <a href="https://glowing.com/glow-fertility-program">ivf</a> cycle can be hugely varied so am remaining realistic, but on the off chance we manage to end up getting more than the 5 we are aiming for - do you think it’s worth asking if we can try going straight into a frozen embryo transfer just to give it a go before resorting to the big surgery etc?

Would love to know your opinions on what you would do or if anyone with stage 4 endo and endometriomas had successful FET without having surgery to remove it all or down regulation prior?
