Is this just a blood clot? Or?

Back in October 11th I had to take Misoprostol because I suffered a missed miscarriage. I bleed for a few days after that. On November the 9th I started my period. It was heavy for a few days. But after that up until today I have been spotting non stop. Sometimes heavier than “lighter”. Well this week since Monday every night/midnight I’ve been cramping really bad along with back pain and sometimes even feeling nauseous. Tonight around 9:30pm I started to feel pressure like I had to go #2. I went to the toilet and as soon as I “pushed” I heard a splash. I quickly check and this was in the toilet. Should I contact my OB again? Or?
