Confused about potential bf.

So me and my ex broke up 6 months ago. We've been together for almost 4 years. I have a friend who went to high school with us and he's been waiting for me ever since. I liked him too, but since we started talking again those feelings resurfaced. He said he is going to take me on a date and ask me out when I get back to my home town. What should I say? He's really sweet and a year younger. I have anxiety and depression, but I haven't told him. Should I tell him what he's getting into? Should I say yes?
