Blocked duct

Anyone have any tips on clearing a blocked duct? I've tried a hot bath with Epsom salts followed by massage and trying to nurse/pump the breast empty but I still have a firm and tender area. Bra is off, not that it was underwire or constricting. I've just finally overcome latch issues and cracked nipples so I was hoping it would be smooth sailing from this point on. Any advice would be appreciated.
