My baby's nappy sticks to his bum😳

My baby boy is 6 days old and when i went to change his nappie i noticed that it was stuck to his bum, i used some warm water to soak it off but it looked sore. I use pampers and change his nappy every 2-4 hours😕 Does this happen to your baby's bum?
