Dentist Phobia

Please tell me I'm not the only one who has panic attacks just thinking about going to the dentist...
I was fine until 15 years ago when I was 14. I had a bad dirtbike wreck and shattered 4 teeth, and they had to be pulled. Went to a new pediatric dentist (mine retired), and he didn't wait for the numbing shot to work. I told him I could still feel everything, and he told me to "get over it, you're lying". He thought I wanted more meds.
He started pulling teeth, and I tried to literally fight back, so he had his two hygienists hold me down while he said "keep your crying, just like the baby you are".
Worst pain ever. Four teeth, no working numbing shot... Ugh. 
More than half my life later and I still freak out whenever I have to go to a dentist. Bad thing? I haven't been in years. The last time I went, I had such a bad panic attack that I passed out in the waiting room.
Right now, I have an exposed nerve. Need the tooth extracted (no insurance, extraction is cheaper than a root canal), and just thinking about even calling the dentist... I break into a nervous sweat and start crying.
Any ideas to ease my fear/anxiety? I really do have to get this taken care of. 
Also, that dentist is no longer practicing.
