Almost passing out...anyone else?

I work 12 hour shifts and I'm on my feet almost the entire time as I'm a nurse and every day I almost pass out, usually during the morning portion of my shift. My doctor keeps telling me it's dehydration but I drink a bottle of water and a cup of tea every morning with my breakfast. My breakfast always contains protein so it can't really be my blood sugars. 
Is anyone else experincing this? 
It's becoming a real inconvenience as I have to leave patient rooms mid-assessment to sit down and wait it out. 
My ears start ringing, a get lightheaded and feel really weak. My blood pressure is normal. I'm just over 17 weeks.
