what could this be?

So my cousin just called me an told me that when she went to the restroom it hurts when she pees )so I think uti) but then she tells me that she is spotting but her period isn't do until a few more days so she asks me what it could be an I'm thinking well maybe ur period just started early, she thinks it could be IB but I haven't experienced that so I wouldn't know, but then she says well may be it could be from her an her BF having to rough sex tmi I know sorry an mind u she is on birth control pills an doesn't use condoms. Sorry tmi I know. she's worried crying an always her first thought is an I pregnant always. Any thoughts what it could be? She's the typ that gets really paranoid an anxiety over everything.
