Please Help Me.. Any Suggestions/Advice Welcome

Ok so, my boyfriend and I are Juniors come this September. We have been together since the end of 8th grade, which is a while. I am very deeply in love with him and my love for him will not stop growing. However he has told me that he feels weird in our relationship and that he feels he is not as good of a boyfriend as he used to be. (Which is true, but is nothing too significant) He feels weird coming over to my house, and spending time with me... even though the last time he came over, he said he did want to be with me and that he was just weird about relationships and that he does love me. He is a very honest and thoughtful person, so I know he is not lying to me, but it is our anniversary on Saturday and I have tried trying to make plans to see him several times and he just brushes them off. (He lives in Massachusetts and I live in Maine, so seeing each other out of school is very rare and a big deal) I am heartbroken as I've spent a long time creating a journal with lots of pictures and memories in it for our anniversary but he can't see it since he won't come over. I feel that I am constantly in a state of wondering whether he is going to decide to break up with me or not.

Any suggestions or advice about how to handle the situation??
