TEEN NEEDS HELP

I've had vaginal issues since I was little. And at 14 my mom wants to make a gyno appointment! I'm excited as I have many questions and I'm worried about many things. She wants me to write a list about the reasons I'd want an appointment but I'm in no way comfortable talking to her about really personal stuff, she knows I have EXTREMELY distressful periods but it's so much more than that, I only want to discuss these things with a doctor or of course you, as it's not personal. Unusual bumps inside my vagina but I'm a virgin, the fact I have HPV but the kind that gives you warts, I've always had a dry red "rash" by my vagina but it doesn't itch, I have some insane cramps, the fact that I've had abdominal issues since I was little, andddd of course my heavy periods. HELP ME OUT SISTERS
