Baby burying face??

Mama's I need help.
My LO is 3 months old and has slept on my chest since birth due to severe colic and reflux (no blankets and I only sleep with one small pillow and no loose clothes, please don't judge me--you have to do what you have to do in order to just make it as a mom sometimes). I'm trying to transition her to sleeping on her own but nothing I'm trying works. She won't even nap in a swing or anything like that. She hates to lay on her back and even worse she likes to BURY HER FACE when she sleeps and it scares me to death. I've tried everything I can think of... white noise, putting her down during a deep sleep, swaddling, swaddling with arms out, elevating the end of the crib mattress, rolling up a receiving blanket under the sheet so she's slightly on her side, warming the sleeping area before putting her down, I've even worn the sheet and put it back on the bed so it would smell like mom.
Tips to transition from co-sleeping, especially when little one hates lying on her back and likes to bury her face?

Are you breastfeeding? 
I breastfeed in the side lying position so baby is already lying down on my bed on her back and I can just slip away when she’s done eating. This would be one way of getting baby use to sleeping on their back. You could do it for nap time and use a monitor....or for bedsharing at first until baby is use to sleeping on their back and then make another transition into a crib

Your going to have to slowly transition her start only with day naps it’s okay if it’s only for five minutes and increase from there. Have you tried the cosleeper that goes on your bed

With a known factor being the reflux and colic issues, I’d talk to your ped about this and see if they have any suggestions
My cousin had a heart issue that showed its face a few weeks after delivery. Full cardiac arrest. He spent time in the hospital for a monthish. He got used to sleeping on his belly while in the hospital, after his incision healed. Also after being on tubes and monitors, he had reflux issues when being on the bottle again, and he wasnt sleeping on his back at home. The doctors made a plan on to how to help him sleep at home, he needed the rest for his heart, and as we all know babies need sleep. They came up a safe way for him to sleep on his belly. From 3m+, he slept on his belly with strict instructions and monitoring. 
I know this case is a more severe, but maybe your doctor will have a solution or recommendation for you.
