What do I do

So I am interning at a hospital in the spring. To get credit I need to have paperwork and complete a physical and all within the next two weeks. My college advisor found me the internship and I went there twice to meet my supervisors. I understand these are working professionals but I received a paper last week that needed to be signed, I emailed them over a week ago and haven't heard back. I sent a follow up reminding them it's due Friday and haven't heard back yet. My advisor is getting mad at me but I don't get what I can do since they aren't replying to me. I don't want to spam them but they agreed to have an intern and aren't taking this responsibility seriously in my opinion. I was gonna have this internship write me a recommendation letter and for references after I finish but they can't even email me back! I understand they are busy and all but now I may not get college credit since I won't have the form completed in time when I gave it to them the day I got it.
