Medicated first IUI ...?

Julie

Hello everyone! I'm fairly new to all this, so I thought I'd seek out some advice. A little background story : my SO and I are 32 and 33 yo and we've been TTC for 2 years. He has a 6 yo from a previous relationship and it was actually an accident. We've had a grand total of 3 MCs (Nov 2016, Feb 2017 and April 2017) so far with no live birth. I've had 2 D&C;'s and developped Asherman's Syndrome after the 2nd one, which is basically scarring inside of the uterus. Mine was completely sealed shut, and I had operative hysteroscopy in Oct 2018 to remove the adhesions. I was cleared to start TTC again right after, but after 7 months, still not pregnant. So I went back to my Dr : we scheduled another hysteroscopy for June 12th (she suspects the scarring reformed) and the cycle right after that (beginning of July), we are trying a first round of IUI.

So! I'm a little confused because my Dr already prescribed 5mg of Letrozole to take on days 3-7 + trigger shot. I have never had any trouble, ovulating on my own, even with Ashermans. And I fell pregnant on the first try with all my previous pregnancies. My SO's swimmers are A1, so no issues there either. The only reason for trying IUI actually is to bypass the "mechanical" barrier of my cervix being scarred, since my Dr thinks it's what is keeping us from getting pregnant again.

So I was wondering why the medication right from the get go? Is it common? Should I skip it? I'm so scared of ending up with too many follicules and this has been such an exhausting journey, I couldn't deal with having to cancel.

That being said, we do have a history of recurring miscarriages, and I really do want to max out the odds of having a live birth in the foreseeable future. I wouldn't mind twins (they actually run in my family on both sides) but I'm scared of high order multiples.

I'm well aware I should ask my Dr about all this, and I will but just wanted to see what you ladies think. Thank you and sorry for the long post!
