Is the end nigh?

Having a proper dilemma so going to start at the top.

So me and my partner decided to try for a baby. I was extremely lucky and fell pregnant very quickly. He was ecstatic. We arranged to rent a flat but at 16 weeks he told me he didn’t love me anymore and we split up. My world fell apart as I loved him so much. I suffered really badly with depression and anxiety due to this to the point I was off work for a while and couldn’t look at anything to do with maternity or baby. Anyway I got on with things and provided and paid for everything that my baby needed with no help from him as it seemed he was too busy booking holidays and going on expensive nights out and weekends away. Not saying that he isn’t entitled to luxuries btw. My little ray of sunshine then arrived. He was at the birth, against my better judgement and did come to see the baby regularly. One night he told me he was coming to visit but instead went and got drunk. He told me he needed help and I was the only one who could do it. We got back together and everything was going good. We planned on getting a house so I started to buy things but when I asked him to buy something small (plates etc) he couldn’t as he was going out!?? Eventually he bought something. My dads recently been admitted to hospital and the unit he was in has a strict no kids policy. I asked him to look after our kid for 2 hours yesterday so I could visit and he refused because he had washings to do. I’m gutted to say the least. I don’t know what to do. He only ever comes up here to stay on his terms. It’s never set days. I feel like this could be the last straw for both our relationship and him being a dad. He’s put alcohol before our child and wouldn’t look after her for a short period of time so how can I reasonably expect him to look after her on access days? Please help!
