What would be reasons as to why a mother and a baby cannot be saved during labor? Why would one have to be chose over the other?

This just happened in my city and a guy had to pick because his wife was hemorrhaging and they could either save the baby or the wife but not both because if they fixed her then the baby wouldn’t survive due to the blood loss and complications but if they took the baby she wouldn’t survive due to the blood loss. 
When I was pregnant and in the hospital due to my extremely severe preE they made me sign a paper saying that if there is a choice I want them to choose my son. They did this because my bp was so high that I had a very high risk Csection.
I think now that I have him I’d choose for them to save me because my son needs his mother.
