Any advice ?

My grandma has a dog and he is very sweet but she isn't caring for him properly he eats but his living situation is horrible he's left out in the rain he doesn't get any attention at all unless I'm there I'm not sure how I should help him she's not old and forgetful or anything like that either she still works and everything so she's well aware of his condition I just want to know how I can help him
