Has anyone ever gotten pregnant from using condom AND pulled out method at the same time?

Let’s say the condom has no holes and it did not break. ALSO, he successfully pulled out without condom getting stuck AND no SPERM in the condom anytime he’s inside of you.... which is is why he pull out then came

There could be a little precum in the condom while he’s penetrating you and that is it...

I’m asking because I’ve seen ladies sharing that they got pregnant from using condoms and the pulled out method at the same time. But, no further informations were given such as ... did he came inside? Were there any holes? Etc.
