Baby growth- please help

Hi all

My baby boys growth was

50th percentile at 4 weeks

25th percentile at 8 weeks

5th. Percentile at 4 months

5th. Percentile at 5 months

5th. Percentile at 6 months

5th. Percentile at 7 months

I am worried a lot ....

He doesn’t like to drink formula or breastmilk

If I add formula into his food he vomits every time

did someone else had any experience like this

Any suggestions please
