TMI - Bloody discharge

Hey guys, so i have been having this problem recently where i have to wear tampons because of bloody discharge. No it is not my period that ended a while ago. Anyways, my discharge has been different for a while now. Its almost as if its flakey. It is a bunch of pieces and not like normal discharge. I am always spotting along with it. Its really embarrassing. Any time i take my tampon out and put it in the toilet the discharge just kind of falls off into little flakes. Could somebody please let me know what this could be. I dont have a uti because i got off medication for that a week ago. (The last 2 pictures are on flash so you can see but my pee looks a lol darker in it)
