Help with birth control!!

Hi, I’m 14. My boyfriend and I have been dating for 2 years. We have been having sex since April but not a lot of the time. Maybe once a month? We always use a condom. I’ve been wanting to stop honestly because I don’t want to get pregnant and I’m afraid to ask my mom for birth control. Also because of the fact that if I’m not ready to parent, I don’t need to have sex. I completely understand if you read this and judge me. But, I hope you know I feel guilty for having it. Well today, my mom told me I have an appointment soon and when I asked why she said it was for birth control. She said she isn’t saying I’m sexually active, she just wants to prevent a pregnancy from ever happening. I’m really scared to go. Like, what happens? And will they know I’ve had sex before? If I tell them I have will they tell my mom? I’m really scared. I don’t want to go.
