Making a prenatal appointment.

I got a positive hpt yesterday & today. My expected period is tomorrow. When should i make my prenatal appointment? I have incompetent cervix and my last 2 pregnancys I had my babies at 21 and 29 weeks, both of them didnt make it. :,( <\3 Ive also had one very early miscarriage. Im so excited yet worried. I have a new family doctor and he doesn't know about the IC yet. The doctor who delivered my last baby said next pregnancy I would need a cervical cerclage, progesterone, and possibly bedrest. So how to i go about telling the new dr about all of this? I want to be seen right aways but what if he wants to wait? Im so scared.. i cant have another loss... i want to be excited but i cant help but worry. Sorry for the long post
