Tubal question

I had my doctors appointment yesterday for being 29 weeks pregnant and I'm planning on having a tubal done after he's born. I asked my doctor about signing consent forms and he said I don't have to because I'm not on state insurance.. Has anyone been told that? Did you sign papers before having your baby?

I know this probably differs from state to state.
