He said he was going to ask me to be his girlfriend but never did

I'm kinda upset and confused right now. I've been going out with this great guy since October, he's probably the most genuine, kind, thoughtful guy I've ever dated. When we first met, he was extremely doting and constantly complimenting me, telling me I was so special, that he couldn't believe he met someone like me. At first it seemed a little excessive but I knew he was coming from a good place and it eventually died down a bit. About three weeks ago, he asked me if I saw a future with him- not necessarily marriage, but immediate future as in a relationship. I said yes of course, even though we'll be long distance while we're at our respective colleges I think he's worth it. He told me he was asking because he wanted to ask me an important question (aka for me to be his girlfriend) next time I saw him. That was three weeks ago and I've seen him twice, we just spent the entire weekend together and had a ton of alone time and he didn't ask me. I'm starting to think that I did something wrong or that he doesn't want to actually date me anymore and I feel like an idiot. It was in the back of my mind this entire weekend and I've been looking for any morning to confirm what I think. I have anxiety and had a really bad panic attack while he was staying over last night, now I'm scared that he doesn't want to be with me anymore because of it. What should I do in this situation? :(
