Looks like a risen! Big bumps that fill up with puss and are extremely sore to the touch. They are harmless I hope it’s something not too serious! Call your doctor explain to them! Hopefully he feels better soon! 💙

That looks super painful! I would try and get him in to see his doctor soon. Until then, maybe let him run around without a diaper for a bit to air it out down there? Also, lots of fluid and baths :) maybe even diaper cream... I'm not sure.
