What is the most effective means for weight/fat loss

I have been struggling with my weight since sophomore year of high school. Before that I was always under 140lbs. Sophomore year(age 17) I was around 175 and now, age 21, I am 215 lbs. I dont understand how could've let myself get this big . I have a few health issues and I was on different types of birth control I feel that could have caused this because it made me crave certain foods. Since last month I am now off of the BC and really want to focus on losing weight. It's hard because my fiance (170 lbs.) Is a lot smaller than me. I'm self conscious in bed and when were around people because it's obvious that I'm a lot bigger than he is. He makes it a point almost daily for me to lose weight. I want to start this year off right with the best plan to burn my fat and lose weight as quick as possible. I want to be under 170 by december.
