Freddy Kruger inside of me

I am writing this on behalf of my sister in law (I have her permission, she is sitting right next to me). She is on birth control, Nexplanon, to help her cramping during her period. She has had it in for a year now but now for the last couple of months she says that when her period starts and then off and on afterwards it feels like Freddy Kruger is ripping out her insides. She said that it isn’t like her regular cramps because these are dramatically different. She can’t currently go to the doctor because she goes through the VA and they are only doing teleconferences. She had an appointment for an ultrasound before all this COVID19 happened but then her appointment got canceled when it hit the fan. I have never experienced or heard of anyone that has had this happen so we were just wondering if any of you ladies have either experienced something like this or have heard of someone who has and what it could possibly be?
