Diaper rash

My baby has this rash. I've tried different creams including A&D, burt's bees, desitin, aquaphor healing ointment, and nystatin (antifungal) cause I thought maybe it was a fungal infection. Is very itchy cause she keeps trying to scratch it. I've tried diaper breaks and putting underwear instead, and frequent baths but is not helping ☹. She never had a rash before. Any suggestions of something that has worked for you in the past???? Helpppp!!!
