Do OBGYNs only count months actively trying before helping?

This is only cycle 4 of trying actively but cycle 16 of having unprotected sex with the mindset of “if it happens yay!”.

I want to get a new OB to see if we’ll need medicine to make baby #2 but I don’t want to be turned away just because we haven’t been using OPKs or tempting all 16 months.

(Background I have very irregular periods but no official PCOS diagnosis)

Do most OBs only care about actively trying months when they say try for 12 months?
