Does this look concerning?

My son has a rash and yesterday a low grade fever at 100.2, he didn’t eat much for dinner or this morning. His fever was gone this morning but by nap time he was at 99.8. I went ahead and gave some ibuprofen and put him down for a nap since he seemed not feeling good. He woke and temp was normal again and he is eating and acting normal, but his rash is now spreading to his face.

He has had a rash very similar to this before and I didn’t bring him in because the Drs always just say “check laundry soap, body wash etc” nothing has really changed. The first rash we figured may be just bug bites(chigger/mosquitoes?) from outside.

Since temps outside are getting cooler now and we had a nice day we brought him outside for little bit. He didn’t crawl much in the grass mostly crawled/walked in the drive way and there weren’t any mosquitoes out since it was cooler outside. Well just the other day we noticed the bumps on his foot and figured maybe ant 🐜 bites? Today they seem to have swollen more and the rash on his body, some of the bumps have a white head/point on them. Idk 😐 I’m gonna get Benadryl but wondering if I should take him in? Or see if it clears up with benedryl..

Current on all his vaccines. Had some done October 2nd.

UPDATE 2: for anyone passing through.

We got a second opinion because his rash exploded and started looking like hives by day 4! Second Dr stated this is NOT HFM disease but a bad allergic reaction to something. Possibly antibiotics or food, anything really, we don’t know for certain. And it covers his whole body, no bumps on hands/feet like you see in HFM disease.
