How to put in a 2 week notice?

So I’m pregnant again with my 5th my youngest is only a year and I want to spend more time with my kids specially my little one before I give birth, also my mental health is not so good I have anxiety and ppd, my manager doesn’t know I’m pregnant again and I would like to keep that away from him as well since I’m only 10 weeks pregnant..
Any advice? How can I put in my 2 week notice? What can I tell them about why I decided to quit?

You don’t have to tell them anything. Just tell them you are resigning and X will be your last day.

“ to whom it may concern. As of (date), that will be my last day of employment. Thank you so much for the opportunities working here. (Your name & signature).”
If they ask, you can disclose whatever you feel comfortable with.

I resigned from my job late last year and was also very early in my second pregnancy. I wasn’t ready to tell my boss about my pregnancy, but we had a very friendly relationship and I wanted to leave on good terms. I gave her my official resignation letter, provided a nice, long notice, and told her  how much I appreciated the opportunity but that it was what was best for my family at the time. She understood, appreciated my honesty, and I know that I left a relationship in tact so I’ll have an easier time getting back into the industry some years down the line when I’m ready to jump back into work. Others are right, you don’t have to provide a reason. Only you know what kind of relationship you have with your boss. In my case, it made sense to share some information.
