What type of questions to ask??

CD 156, TWO PERIODS THIS YEAR AND LAP SURGERY.

I finally have a phone in appointment with my family doctor tomorrow and I’ve never been very good at appointments with him. I was wondering what type of questions to ask tomorrow regarding the topics above and the difficulties we have faced this past year TTC. I have PCOS and endometriosis that he diagnosed me with and I really want to discuss actually doing the lap surgery to be properly diagnosed and see how bad everything is.

Any kind of advice you would help. Thank you!
