Post sex bleeding

So I've been having sex with my boyfriend for over three months now and nearly every time there is some spots of blood left over. We've tried everything from more foreplay, to different condoms, and it still happens... he has to wear size large condoms to put it into perspective but I don't think that's the issue. There's no way I have any STD's and there has been no itching or anything unusual so no infection. I didn't bleed the first time we had sex and I only started bleeding after sex into month 2. Between then the only change is my anxiety medicine has been increased. Any ideas??
