2 year old waking to be rocked, HELP!

This might be jumbled up so bare with me.

My daughter is two and she has always been a great sleeper, until now. I stick with the same routine that she has had since she was a couple months old. I have tried to adjust her nap time to a shorter duration, I've tried letting her go to sleep later than her bedtime, I've even tried cutting nap time out completely. She wakes up at least once a night, sometimes up to three times, to be rocked and begging for a bottle then she will go right back to sleep. I know I need to cut the bottles out completely and maybe that's the problem. She doesn't have anything but water a couple hours before bed, she doesn't have TV or screentime a couple hours before bed. I sing and read books 45 mins-1 hour before bedtime in her room with a nightlight to wind down. I'm at a loss of what I need to do. If I sit in the rocking chair while she's in her crib, she thinks it's playtime and WILL NOT go to sleep so I have to leave the room until she starts crying then do it all over again (rocking for a couple minutes until calm then putting her down) WHAT DO I DO? I'm loosing my mind and I feel like I'm failing her!
