5 years no ring.... please read description

We've known eachother for almost 17 years. Been together almost 5 years in December. He knows how important marriage is to me. I told him at the beginning I wasn't interested in being together for 5 years and not being married..in those exact words... yet here we almost are. Still no ring. He has never said he wouldn't. He actually said for years I had to quit smoking first. I did over a year ago. Some things need work and are annoying but he's good to me and the kids. Doesn't cheat, hit, drink or smoke. I don't know what to do. I feel like I'm not worth it to him.
