Account sync with new phone?

My phone is acting up lately and I know I will need to replace it soon. Will all my account information, period dates, etc. Carry over when I download the app on my new phone?

I was using a different tracking app last time I got a new phone and completely lost all my info! Luckily I wasn't sexual active at the time, so I wasn't too worried about tracking things closely.

I dont remember any login info for this app either since i am just automatically logged in. Is there a way I can double check this before I get a new phone?
