Sore niples

The past few months I've started to get sore nipples with my period. I know it's a common pms symptom, but it's new for me. This month my right nipple has been especially sore and I've noticed the shape feels different. Has anyone else experienced something similar? I'm going to ask my doctor about it, but how urgently do I need to make the appointment?
