Pre-conception appointment?

Okay so hubby and I plan on TTC within the next year. I'm super nervous about it since the best time for us is a short window of 5-6 months. I use NFP as my BC, so I already am very aware of my fertile cycle and such. I'm wanting to go to a preconception appointment to:
1. Give me a rundown of everything I need to go to make sure I'm in prime physical condition to conceive (important because I'm on the border of overweight-still healthy weight for my height, and also I have lupus. But I already know I don't have the miscarriage causing antibodies)
2. I want to get a general work up to see if I have any of the common conditions that make it harder to TTC.
3. I'm super anal about doctors approval and need to get sound medical advice to feel comfortable with my decision.
...
However, I am a little worried about being judged because I am young. I'm 21, but as I said I have a condition that will just get worse with age, so I'm wanting to start when I am at my peak of able bodied-ness... Plus we're working out the finances and looks like we could do this despite being young. (Hubby is almost done with school and will be a teacher- guarenteed job because of all the shortages in our state)
...
Does this sound like a good idea? What do I ask/say? I know for my insurance I need a referral from my GP, but that shouldn't be a problem, but I'm nervous because my GP always seems so judgemental whenever I come in (super regularly) due to being young. Comments like "oh you're too young for that, or for this, etc." and keeps trying to get me on different birth control so that I "don't ruin my life with a baby so young". We've made sure we could afford an unintentional baby, but as far as actually planning one we want to be more prepared.
