exploding poop

Recently baby girl has been exploding out of her diaper. Ill change her and 5 mins later, she has poop up to her neck, this happens sometimes 1-2 a day. It has ruined several different outfits even though i have washed them more than once. I have only been using baby detergent so far but im wondering what else would be safe to use? Can i use bleach? Stain remover spray? Bleach for colored clothes detergent? Or anything else?
