what do you keep in your diaper bag?

What are necessities for your diaper bag? 
I feel that this is very important, and I'm still shopping for little things I need yet. Let us know!

About 6 Diapers, wet wipes, butt cream, pacifier, 2 toys, 2 extra outfits. 1 being for warmer weather and 1 for colder. A small blanket. Bottle of water for me. That's pretty much it :)

In my newborn diaper bag (since now with my toddler I just grab a diaper and wipes) I carried:
Wipes
Diapers
Butt cream
Change of clothes
Light blanket
Burp cloth
Nipple cream
Extra breast pads 
Tissues
Baby nail clippers
A snack
A toy

Diapers, wipes, pacifiers, extra onesie or outfit, distracting toy, teether of some sort, snacks for mom and baby, burp rags, a travel changing pad.. I keep a lot of stuff in my bag but depending on age these are my main things.
