What is it? Periods?

Something happens very badly with me today today after pee in washroom when i clean i see blood on tissue and small clot too befor 12 days of my period..i am very confused i search on internet but didnt get the point. I want to conceive but very bad :( plz help me 1.5 year passed this time was using Preseed too and i am gaining my weight very quickly i m worried help me
