help please

So my periods have been fine for two years now I had bad problems at the start then it calmed down for the past year only lasted for five days at the most now in September I was on heavy for 12 days and from end of October to November this is now my 14th

I've been the toilet and this has just fell out me 

Anyone else had this

I sometimes have no bleeding throughout the day or very light then it goes very heavy later on
