Need Advice

I need some advice...My 2 month old was accidentally given an extra dose of a vaccine. We had switched doctors and he was given it at his old doctors and his new doctors, only a month apart. The issue occurred because the new office never scanned in his file that was faxed to them. 
The appointment was on Monday and I realized the issue that night and on Tues talked to a nurse and left a message for the doc to call me, he still hasn't called. I called again today and they said he would call today but still hasn't called. I also left a message for the practice manager and haven't heard back. 
This is what we have decided so far but I wanted to see if this issue has happened to anyone before and/or if you have any advice on how to handle it. 
We decided to:
1. Switch doctors (unfortunate since we just switched)
2. I want it in writing that he was mistakenly given it at the wrong time in case there is an issue later. It's part of a 3 dose vaccine and I don't want to run a future preschool to say he didn't get it when he should have or if we have to get a 4th shot, I want to make sure insurance covers it since it would be an "extra" dose
Any other advice? I'm really frustrated and upset about the whole situation. (Please only helpful advice, no comments saying I should have kept better track of the vaccines myself, etc. I'm a brand new mom and this is all new to me and I already feel bad enough about the situation)
