how do you have a "surprise" pregnancy

I'm legit curious. I mean, I'm not on BC and we use condoms properly and have never had a problem. I'm just curious. Does BC fail? Or are people just not careful enough? I wouldn't mind a surprise since we keep going back and forth if we'd like another....I think we're just lucky/too careful lol.
